3. Complaints: If you believe your privacy rights have been violated, you can file a complaint with us or with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.


Your Rights to Disclosures of PHI  

Under the Health Insurance Portability Act (HIPAA), individuals have the right to receive an accounting of disclosures of their protected health information (PHI) made by ReNew Recovery Cafe in the six years prior to the requested date, with certain exceptions.


How to Exercise This Right

1. Submit a written request to ReNew Recovery Cafe.

2. The request specifies the time period for which the accounting is sought, up to six years,

ReNew Recovery Cafe must provide a written accounting of disclosures, including the date, recipient, and purpose for each disclosure, except for those made for treatment, payment, and healthcare operations, among other exceptions.
